I will hold the following personal data:
Personal and contact details; your name, date of birth, address, email address, telephone number(s)
Emergency contact; next of kin and G.P. contact details
Consultation and session notes; these may include medical history, family and personal social history that you choose to share with me
Your personal and contact details, emergency contacts, consultation notes, and session notes will be held separately. The session notes will be coded as to remain anonymous and only identifiable by me
How that data is used?
I hold the personal and contact details in the event that I need to confirm your identity should I need to contact your G.P. or next of kin, or a request of access to the information I hold by you or representative, or if required by law such as a court order.
I also use the personal data as a means of communicating with you
The consultation and sessions notes are to support my work with you in ways that enhance the therapeutic work in our sessions and for supervision purposes. Please note that my supervisor is also bound by confidentiality
In the event that I am incapacitated for any reason, as my professional will states, your contact details will be shared with my supervisor or other named person, in order to inform you of my circumstances
How is the data stored?
Your data will be stored in the following ways:
Mobile – This is password protected
Emails – I use Gmail which is password protected
My PC has anti-virus software installed which aims to protect my PC from viruses, spam, spyware, and malware
Consultation notes – This will be in paper form and will include your personal details (as mentioned above including your name, D.O.B., address, and family and personal history) and will be kept in a locked filing cabinet. This will be held separate from the session notes
Session notes – This will be in paper form and these will be kept in a locked filing cabinet
Length of time data is held
I hold your personal data including the consultation and session notes for a period of seven years after which I will destroy or delete
I will also hold any written communication between us for a period of seven years after which I will destroy or delete
Should you not want to go ahead with any counselling sessions after the initial consultation, I will delete your contact details from my list
Request for data held
You can request access to the data I hold on you verbally, through email or written request. I will endeavour to provide you with the information requested within 28 days.
You can request that the information held on you be deleted or changed if it is deemed to be incorrect
Should you wish to complain about the way in which your data is being handled then please discuss this with me. If it is not resolved satisfactorily, you have the right to contact the Information Commissioners Office https://www.ico.org.uk
Confidentiality
I work and abide by the ethical framework of the British Association of Counselling and Psychotherapy
My confidentiality policy are set within the terms of our signed agreement when therapy commences
If I become aware of any accidental or malicious breaches of your personal data being accessed by a third party, I will notify you within three days
